Student Request Form
Request Policies
Students must read and follow these policies carefully in order for their request to be considered.
- Every student is allowed absences in every class (5 for daily technique class), as designated on each syllabus. In the case of daily technique class, if you have a very large number of total absences (because of illness, injury, or some other extenuating circumstance), and after consulting with your teacher, you may need to consider "reduced credit". In every class, regardless of your total absences, students are required to fulfill course requirements and execute quality work.
- If you know that you will be absent for an upcoming event, such as an audition or workshop, it is expected that you will make a request and receive approval for the absence before you leave. You need to provide details about your scheduled event including the date and times. If it is a workshop or outreach opportunity, please document the quality/rigor of the program so that we can determine if it is an appropriate replacement for your normal classes.
- If you are absent because of illness, injury or some other emergency, you are required to make a request within one week of your return. In the case of illness, you are required to include a doctor’s note. If you are unable to scan your doctor’s note, you may put a hard copy in my box in the administrative office.
- If you do not follow these policies, your request will either be denied or it will be sent back to you requesting more information. If we need more information, you will need to resubmit a request that provides that information. This will require additional time of you and us.
- If your absence occurs during the last week of the semester, you need to submit your request immediately upon your return.
- Once you have received an approval, it is your responsibility to show your approval to your teachers. Note: You should show your approval to your teachers within one week of having received it. Note: The deadline for showing ANY approval to faculty is 12 noon on Wednesday, April 25th. If you do not do this, you will not be given credit for your missed attendance.
