Policies & Forms

These policies and forms are designed to be a resource for information concerning requirements and policies. The faculty reviews these documents on an on-going basis to make changes they deem necessary to reflect current practices, expectations, and changes in University policies.

  • All Ballet BFA majors are expected to adhere to the dress code for each type of class.

  • The Curriculum Exception Form is used by students to propose course substitutions for transfer classes or other University of Utah coursework.

  • Emergency Response Coordination for SOD classes held during normal business hours is handled by the Administrative Manager. Evening and weekend emergency response during SOD productions is handled by the Production Director. If a student, faculty, staff, or community member is in imminent and immediate danger, please call:

  • Students and instructors in the School of Dance are expected to follow the SOD Grading Policy.

    Students registered for courses in the School of Dance may appeal a final grade in accordance with University Policy 6-100. Note that students must demonstrate that the grade was either a mistake or “arbitrary and capricious.” The appeals process can be found as part of the SOD Grading Policy.

  • Teaching Independent Study courses is not required of faculty members. Offering Independent Study courses can create a significant amount of additional work for faculty, but faculty do not receive teaching load for these types of courses.

    SOD faculty must receive Director approval to offer independent Study courses. Director approval must be received prior to a faculty member committing to a student and prior to student registration for the course. Director review is important to 1) ensure the Director can ascertain a broad view of curricular needs, ensuring that Independent Study offerings do not overlap with the existing curriculum, and 2) to ensure the Director can accurately assess faculty workload.

    Faculty members may request Director approval to offer an Independent Study course through the Faculty & Staff Resources page.

    Undergraduate Students

    If undergraduate students request Independent Study courses, faculty should encourage students to connect with an academic advisor and explore other options to fulfill degree requirements.

    Graduate Students

    Of the total 60 credits a candidate must accrue to earn an MFA, only 10 may be from independent studies.

    Graduate students may also take any undergraduate dance course offerings that support their research. To receive graduate credit for these classes, the candidate should register for a graduate independent study (DANC 6951). (Note that undergraduate course numbers may not count toward graduate degrees.) The candidate should first inform the Director of Graduate Studies of their intent to take an undergraduate course for credit and then work with the instructor of record to ensure appropriate completion of course requirements. 

  • Any student who is anticipating a long-term absence of one or more semester must complete the Leave of Absence form for faculty approval.

  • Lockers can be reserved in the MCD locker rooms by completing the Locker Agreement.

  • All School of Dance students who are cast in productions must abide by the Performer Etiquette and Conduct Code.

  • Students needing a Permission Code for a School of Dance class should request that using the Permission Code Request.

  • Currently matriculated School of Dance majors who are offered professional performance contracts may be eligible for the SOD Professional Track. The Professional Track is by application only and must be approved the SOD Director (or delegate). Once accepted into the Professional Track, students may be permitted to receive credit for professional work by registering for “Professional” courses. Acceptance into the Professional Track is good for one academic year. Students will be required to re-apply for the Professional Track each academic year.

  • Students must complete the Student Information Sheet at the beginning of every academic year.

  • Current students should use the Student Studio Request form to reserve studio space. Students should also review the Studio Scheduling Policy and complete a Building Access Form (if necessary).
