Guest Lecturer / Artist
The documents below will need to be sent to the Guest Lecturer / Artist before they come to class. The guest will need to complete all paperwork and submit to the Administrative Officer before attending the class. Please check with the Administrative Officer before reaching out to guests to make sure funding is available for your class.

Emergency Response / Incident Reporting
Emergency Repose Coordination for School of Dance classes held during normal business hours is handled by the Administrative Officer. Evening and weekend emergency response during School of Dance productions is handled by the Production Director. If a student, faculty, staff, or community member is in imminent and immediate danger, please call 911. School of Dance Emergency Response Plan can be found below.
For Incidents / Accidents that happen either in the School of Dance, in another building related to School of Dance classes, or elsewhere in regards to School of Dance classes, productions, or faculty research, please fill out the Incident / Accident Form below and forward the confirmation email to the School of Dance Administrative Officer and Administrative Assistant. This form must be completed by the faculty / staff member that witnesses the incident--not the injured party. For questions regarding this form, please reach out to the School of Dance Administrative Officer or Administrative Assistant.

Absence Reporting
All School of Dance Faculty and Staff absences must be reported using the Absence Report the link below. Planned absences must be requested in advance and approved by your direct supervisor and/or the School of Dance Director. In cases of unforeseen absences, absence reporting must be completed within 3 days of the absence.

OSHA Workplace Requirements
This memo is sent regarding the annual posting required by OSHA. The OSHA Form 300A Summary must be posted throughout the University in order for the University to comply with Occupational Safety and Health regulations.
The attached notice is to be posted today through April 30th. Please post the attached notice in conspicuous places where notices to employees are customarily posted. Steps should be taken to ensure the notice is not altered, defaced or covered by other material.
A recorded injury or illness under the OSHA system does not necessarily imply that management or the worker was at fault, a violation of an OSHA standard has occurred, or that the injury or illness is compensable under worker's compensation.
